IMPORTANT - If you are Paying for yourself to enter, complete SECTION A contact details, only. If you are paying for someone else to enter, complete both SECTION A & B contact details. Please make sure ALL information is included. If you are paying for someone else to enter it is your responsibility to ensure their full contact details are entered and are accurate

 

NOTE: If you are paying for more than one entrant you MUST repeat the form filling and payment process for each entrant
